Medicine, Made for You is a plain-language guide to precision medicine written for patients, families, caregivers, students, journalists, advocates, and curious general readers who keep encountering pharmacogenomics, genomic profiling, polygenic risk scores, and gene therapies in headlines and clinical conversations and want to understand what the words mean, what the tests actually do, and what to ask for. The book takes no biology or medical background for granted, builds the science from first principles, and grounds every chapter in a sustained narrative inside Meridian Health Partners, a fictional twelve-hundred-bed academic health system in the Texas Medical Center.
The book opens with the structural failure that motivates the field. The average patient does not exist. Clinical trials measure outcomes across populations and report means; medicine practiced from those means treats every patient as if they were the average, and the patients who are not average-who metabolize a drug too quickly or too slowly, who carry a variant the trial never measured-are the patients who account for the 250,000 to 300,000 preventable adverse-drug-event deaths that occur every year in the United States. Robert Tillman, a school-bus driver in Galveston County who survives a near-fatal warfarin reaction, opens the book; the system that almost killed him is the system the rest of the book is about transforming.
From that opening, ten chapters develop the science and practice of precision medicine in the same voice: explanation before significance, mechanism before metaphor, and the patient before the policy. Chapter 1 establishes why one-size-fits-all medicine fails real people. Chapter 2 is the science chapter-genomics, DNA sequencing, biomarkers, and epigenetics in plain English. Chapter 3 covers pharmacogenomics and the preemptive PGx panels that prevent cases like Robert's. Chapter 4 develops comprehensive genomic profiling in cancer and the targeted therapies it has unlocked. Chapter 5 covers whole-genome sequencing for rare disease and the first generation of CRISPR gene therapies, including Casgevy. Subsequent chapters address polygenic risk scores in cardiovascular disease and diabetes, precision oncology, microbiome and metabolomic profiling, the data infrastructure-electronic health records, biobanks, the NIH All of Us Research Program-that makes the field possible, the access and equity questions that determine who actually benefits, and the policy frameworks that govern privacy, insurance discrimination, and informed consent.
Four recurring characters carry the arc. Dr. Amara Osei, the chief genomics officer fighting institutional inertia. James Kowalski, the pharmacist who becomes the institution's preemptive-PGx advocate after watching a near-fatal reaction unfold. Sofia Reyes, the navigator who sits between the genomics laboratory and the families waiting for results they cannot read. Dr. David Chen, the skeptic whose worldview shifts when his sister's lung cancer reveals an actionable EGFR mutation. Each chapter opens at Meridian, then spirals outward to the science and institutions reshaping medicine globally. The characters are fictional. The science is real.
By the final chapter, readers will understand what precision-medicine tests to ask for, what results actually mean, how to advocate inside a healthcare system that has not yet caught up to its own science, how to evaluate access and equity questions, and how to participate informedly in the conversations-clinical, ethical, and political-that will determine who benefits from this era of medicine.
